Elmbrook Grooming is currently accepting new clients under 50 pounds in size. If you're currently a client of the Elmbrook Veterinary Clinic with a dog over 50 pounds, please give us a call to inquire about grooming options.
Grooming Waiver & Consent Form
All Grooming Clients are required to fill out a Grooming Consent Form.
This agreement shall apply to all grooming visits by your pets at Elmbrook Veterinary Clinic Grooming (EVC Grooming): A current waiver must be on file in order for pets to be groomed.
This agreement is in place for one year and must be signed annually.
